Do you have a nonprofit organization that serves the people in the Franklin County area? The nonprofit organizations we support provide health and human services that strengthen families, nurture and protect children, assist the elderly and disabled, and provide emergency assistance.
Our application process is rigorous because our donors deserve to know that their funds are being well-spent. It starts with an application, due on October 15, 2023. View the checklist below, and get started when you’re ready.
Application Requirements
Before you submit your request for funding, please make sure you have all of the following:
- Completed Agency Application
- Current list of Board of Directors, including contact information and business or organization affiliation
- Most recent 990 tax return*
- Operating budget for current year or projected for next year
- Copy of most recent Audited Financial Statement**
- Program Results for 2023
- Program Goals for 2024
- Federal Employer ID Number (EIN) documentation
- Supplemental Fundraising request for each 2024 planned fundraiser
*If not applicable, substitute most recent year-end financial statement
**If you were not audited, attach reviewed financial statement. If you are submitting this application for the first time, please attach a Federal IRS Letter and Tax Exemption Letter.
Applications that do not contain all of the required documentation listed above will not be accepted by United Way for review. Please submit one copy of your completed application by October 15, 2023.
Application Glossary
Number Served: This number is the total number of people served with each person being counted only one time. Ex. Jane Doe and her two children come to organization X on February 18. On that day they receive xyz services. They are counted as 3 people served. They return for abc services on March 19. They are not counted in the total # served again on their second visit. (This information is requested on page 3.)
Units of Service: This number accounts for each service provided to each person served. This number will be higher than the total number served. Ex. Jane Doe family from above received 3 units of service their first visit and three units their second. This is a total of six units of service. (This is requested on page 3.)
Administrative Cost: Use the Administrative Cost % Calculation Worksheet below. You will need your most recent 990.
